How it works

No forms to work out on your own, no waiting rooms, and nothing to pay. Here is what actually happens.

You tell us who needs help

Three questions: their name, and which county they live in. Two minutes on a phone. You can also just call us and we will fill it in for you.

A navigator calls you back

Within two working days. A real person, not an automated system. They will ask what is happening and what would make the biggest difference this week.

We work out what is going on

A conversation, usually about an hour, at your home or over the phone. We look at eight areas where things tend to go wrong — wandering, medication, money, whether you as the carer are running on empty.

You get a safety plan and an Emergency Workbook

A plan written for your situation, and a workbook with a magnet for the fridge so that if paramedics ever come to the house, they know who they are helping.

We make the calls

When you need a service, we call them. You do not get handed a list of websites. We contact the provider, we tell you what they said, and we keep track of it.

We check in

After a week, after a month, and then regularly. If something changes, tell us and we will move faster.

What it costs

Nothing. This is a Manatee County program funded to serve county residents, and there is no charge to you at any point.

What we will not do

  • We do not provide medical care or diagnose anything.
  • We do not replace 911. If someone is missing or in danger, call 911 first.
  • We do not share your information with anyone without your say-so.
