If you are not sure whether this applies to you, ask anyway. We would rather tell you than have you not call.
Before you rule yourself out

You can refer yourself. So can a neighbor, a church, or a paramedic.
There is no charge and no means test. We do not ask what you earn.
Earlier is easier. The whole point is reaching you before the emergency room does.
The actual criteria
Eligibility is self-attested — you tell us, and we take your word for it. There is nothing to prove and nothing to upload.

The unpaid family member, partner, or friend. Not a paid professional carer.
The awkward questions
