What we do with what you tell us

Written to be read, not to protect us. If anything here is unclear, ask and we will explain it.

Dementia NavHub is a service of InformedCare360, and shares its privacy, legal and HIPAA policies. This page explains in plain words what this system does; the binding document is the InformedCare360 policy set. Where the two ever differ, the policy set governs — tell us and we will fix this page.

Who can see your information

  • The navigators at the Manatee County Hub who are working with you.
  • Nobody in another county. The system is built so that is not possible, not merely discouraged.
  • A first responder who scans a bracelet, and then only a short safety summary.

What we record

What you tell us in the assessment, what we did about it, who we contacted on your behalf, and what came of it. We keep this because it is how the next navigator picks up where the last one left off, and because the county has to show what the funding paid for.

What we do not collect

  • Social Security numbers.
  • Medicare or insurance numbers.
  • Medical records or uploads from your doctor.

These are not policies we ask staff to follow — the system has nowhere to put them.

Every look at your record is logged

When a member of staff opens your record, that is recorded: who, when, and from which screen. If you ever want to know who has looked at your information, ask, and we can tell you.

How long we keep it

For as long as we are working with you, and then for seven years. That is the standard for health-adjacent records. The exact obligation for this program is still being confirmed with the county, and we will update this page rather than quietly change it.

Artificial intelligence

We use AI in two narrow places: summarizing a list our own system has already produced, and describing a navigator’s caseload for the day. It is never given your name, your address, or anything you wrote — only counts and codes. It never decides anything, and every screen works with it switched off.

Asking us to stop

You can withdraw at any time and we will close your case. Tell your navigator, or call the Hub.
