Free · Manatee, FL
We help you make a safety plan, put an Emergency Workbook on your fridge for first responders, connect you with local help, and check in so you are not left on your own.
If someone is missing or in danger right now, call 911 first.

What you get
Not a directory and not a leaflet. These are the things a family actually leaves with.
Written with you — what to do if your loved one wanders, becomes upset, or you cannot keep going.
A book for your fridge, with a magnet, so first responders can find what they need in seconds.
So anyone who finds your loved one can identify them and reach you straight away.
A navigator who knows your case. You never have to explain from the start again.
How it works
No forms to work out on your own, no waiting rooms, and nothing to pay.
Three questions, about two minutes. Or call us and we will fill it in for you.
Within two working days. A real person, not an automated system.
One conversation, about an hour, at home or on the phone.
A safety plan and an Emergency Workbook, written for your situation.
You do not get handed a list of websites. We call the provider and tell you what they said.
After a week, after a month, and then regularly.
Data minimisation
This is not a policy we ask staff to remember. The system has nowhere to put these things.
A Social Security number
A Medicare or insurance number
Copies of medical records from your doctor
Eligibility is self-attested. You tell us what you are comfortable telling us, and every time a member of staff opens your record it is logged. How we handle your information
Questions
Nothing. The Hub is funded by a Manatee County grant to serve county residents, and there is no charge to you at any point.
No. If memory loss or confusion is making life unsafe or unmanageable, that is enough to talk to us. We do not ask for medical records.
A navigator calls you back within two working days. If you tell us something is happening right now, we move faster.
Names, contact details, and what is going on. We never ask for a Social Security number or a Medicare number, and we do not take copies of medical records.
Your navigator and the Hub staff for your county. Nobody outside it, and we do not share your information with other agencies without your say-so.
No. If someone is missing or in danger right now, call 911 first — then tell us, and we will pick it up from there.
You may not be the family
Each of them has their own way in, and their own account.
