Terms of use
The short version: this is a free county navigation service, it is not medical care, and it is not an emergency line.
Dementia NavHub is a service of InformedCare360, and shares its terms of use. This page explains in plain words what this system does; the binding document is the InformedCare360 policy set. Where the two ever differ, the policy set governs — tell us and we will fix this page.
This is not an emergency service
Nothing here is monitored around the clock. If someone is missing, hurt, or in immediate danger, call 911. Do not use the referral form or a message thread to report an emergency.
This is not medical advice
Navigators are not clinicians. Nothing in a safety plan replaces what a doctor tells you, and we will never suggest changing medication or treatment.
What we promise
- To call you back within two working days of a referral.
- To contact services on your behalf rather than handing you a list.
- To tell you what a provider said, whether or not it was the answer you wanted.
- To keep your information to the people working with you.
About referrals to other organizations
We check that a Resource Partner serves your area and confirm their details once a year, but they are independent organizations. We do not employ them and we cannot guarantee their service. Tell us if a referral goes badly — it changes who we send people to.
Eligibility
This program is funded to serve Manatee County residents. If you move out of the county we will help you find the equivalent service where you are going.
